      I don't believe that there is, however you could create one

      yourself with not too much trouble. A couple of questions. Will

      your printer print on paper that size? Or are you going to cut

      down a regular size piece of paper?

      If you are going to cut down a regular size piece of paper, then

      you may want to look at setting up a table to type your

      information in. I find the easiest is to setup the table so that

      it is two cells wide. I would set the two cells to be 3.75" wide

      with internal left margins to support the 3 hole punch.. I would

      set the height of your rows to be 6.75" high again with internal

      margins to support where you want to start typing on the page.

      If you are planning on printing a date on each page, and a

      subject on each page or have a set layout like that, then add

      form fields in the proper places and protect the document so that

      they don't float around on a page. I did a cookbook just like

      this only my pages were 6.5 x 8.5 so that all I had to do was cut

      an 8.5 x 11 sheet of paper in half.

      I hope that gives you an idea of how to get started.
